v The Application and Learning Contract MUST be completed and approved before the start of the internship.

v An evaluation questionnaire will be sent at Mid-term to the Internship Supervisor to complete and return to the Internship Coordinator.
v Students MUST attend THREE (3) Career Services events before you conclude the internship in order to receive academic credit. These THREE events can be: WORKSHOPS, SEMINARS, JOB FAIRS, OR CAREER COUNSELING APPOINTMENTS. The Career Services Office will date and signature a form that I have provided for them to prove that you have participated in the three events. You can go to the Career Services web site for dates/times of the events: career.uri.edu and they are located in Roosevelt Hall, second floor, #874-2311. (This requirement is waived for the summer and holiday break internships.)
