STEP 3:

Turn in your Record Book

This is a general record book that all 4-Hers should fill out. It is 2 pages front and back and should be filled out and turned in every year. This form provides a place for 4-H Member's to list their project accomplishments, life skills learned, participation in 4-H contests and events, community service activities, school and community involvement, leadership, public presentations, and special awards. The 4-H Record Book replaces the yearly summary sheet. (4-H Members enrolled in animal science projects must also complete the 4-H Animal Project Record).

This is a record form for those 4-H Members raising animal projects. It is 2 pages front and back and should be filled out and turned in every year with the 4-H Record Book. The 4-H Animal Project Record provides a place for 4-H Members to keep their animal inventory, health records, breeding and birth data, time management, and feed, bedding, equipment, and project cost information.

These two pages are for horse project members and should be attached to the Animal Project Record. The Show Record Sheet may be used with any animal project.

Make sure your records are signed by the right people. Have your leader or parent check it over for you. Turn it in, on time, to the 4-H Office.
