All the above information must be entered to process a lost ID card. Once a card has been report lost/stolen you must come to the Campus Access Office to get a new card the next business day. In the event you find your ID card you can reactivate the card at the Campus Access Office. Anyone attempting to use a card which has been reported lost or stolen (including yourself) will have the card confiscated.

The charge for a new card is $10 Dollars.
