The premium rate
for basic life insurance is based on your age and contract
salary.
See examples below for an employee whose contract salary is $30,000.

Employees up to age 54 will pay:
A 26 pay period employee will pay:
$30,000 x .1828 = $5.48 per pay period x 26 = $142.58/yr
A 20 pay
period employee will pay:
$30,000 x .2376 = $7.13 per pay period x 20 = $142.60/yr

Employees age 55 and above pay:
A 26 pay period employee will pay:
$30,000 x .1934 = $5.80 per pay period x 26 = $150.85/yr
A 20 pay period employee will pay:
$30,000 x .02514 = $7.54 per pay period x 20 = $150.84/yr

Supplemental (Optional) Life Insurance

Example:
You are 33, and you work 26 pay periods per year, your contract salary is
$30,000 x .03740 = $1.12 per pay period = $29.12

You are 33, and you work 20 pay periods per year, your contract
salary is $30,000 x .04860 = $1.458 per pay period = $29.16 a year |
