TITLE: Coordinator,
Medication and Alcohol Intervention Program
DIVISION: Academic

REPORTS TO: Principal Investigator
GRADE: 12
SUPERVISES: N/A
BASIC FUNCTION:
Responsible for educating elderly Rhode
Islanders in senior outreach locations and through a telephone call-in system
concerning their medication and alcohol use and misuse.
ESSENTIAL DUTIES AND
RESPONSIBILITIES:
Evaluate
patients' individual drug therapy and based upon this evaluation, instruct
patients on appropriate use of therapy.
Provide
verbal reinforcement for patients as well as individual written programs to
improve medication use for the elderly.
Counsel patients on a regular basis through
one-on-one interactions.
OTHER DUTIES AND
RESPONSIBILITIES:
Report to the project director of the Medication
and Alcohol Misuse in the Elderly Housing Units Program.
Direct
the elderly to appropriate agencies for long-term follow-up when deemed
necessary.
LICENSES, TOOLS AND
EQUIPMENT:
Must be able to be certified as a registered
pharmacist in
ENVIRONMENTAL
CONDITIONS:
This
position is not substantially exposed to adverse environmental conditions.
QUALIFICATIONS:
B.S.
in Pharmacy and/or Pharm. D. required, as is the
ability to be certified as a registered pharmacist in the State of
ALL REQUIREMENTS ARE
