Re:              Disease Management Programs

 

On July 1, 2007, UnitedHealthcare will begin offering Disease Management Programs to help State of RI employees better manage their health care needs for the following three conditions:  Asthma, Diabetes, and Congestive Heart Failure.  This new Disease Management Program is voluntary and all personal health information is treated with the highest level of confidentially.

 

Employees and dependents with the above conditions may receive educational materials in the mail or may get a call from a specially trained registered nurse who will become their health advocate if the member elects to participate in the program.

 

 

The Disease Management educational materials or nurse will:

·        Help explain benefits and assist in accessing services.

·        Provide one-on-one health care information, guidance and support.

·        Help prepare the member for discussions with their doctor.

·        Provide support for following their doctor’s treatment plan.

 

 

As part of this program, employees will automatically receive real-time access to resources to help:

·        Get important information about their health condition.

·        Research and understand available treatment options.

·        Make important healthcare decisions and more effectively manage healthcare needs.

·        Improve their health and quality of life.

 

 

The State of RI UnitedHealthcare benefit plan also includes a number of health improvement programs and resources accessible on the web. These programs cover topics such as weight loss, fitness, and smoking cessation and are available to all members.
