MATERNITY LEAVE
- A doctor's note is required with predicted delivery date.
- Sick leave may be discharged when a physician's
disability statement is provided.
- A request for leave and doctor's letter should be
submitted to the employees' department at least 2 weeks
prior to the leave begin date. The employees' department
will then prepare a USP-21 and forward it to the Office
of Human Resource Administration.
- The employee has the choice of whether to discharge any
or all vacation time and personal time.
- There is a maximum of 1-year leave without pay with paid
benefits.
- Once the leave dates have been set, an early return
requires 30 day written notice to the department and the
Office of Human Resource Administration.
