I. APPLICABLE TO:
All University employees.
The Office of Human Resource Administration is responsible for coordination and
implementation of this
Supervisors, of terminating employee or employees transferring from the
University to another state
agency, will contact the Office of Human Resource Administration to schedule interview.
The Office of Human Resource Administration will afford any
terminated/transferring employee a personal
exit interview with the Director of Personnel Services, upon request.
In accordance with the State of Rhode Island General Law 28-5.1, an Exit
Interview Program has been
established in order to assure that terminating/transferring employees are not leaving because of discriminatory
circumstances. All terminating/transferring employees have the option of an Exit Interview with the State of
Rhode Island Equal Opportunity Officer or they may opt to send it to the State Equal Opportunity Office, the
Exit Survey Inquiry.
Terminating/transferring employees upon signing terminating/transferring papers
in the Office of Human
Resource Administration will be given two forms:
(1) Rhode Island State Equal Opportunity Office CONFIDENTIAL EXIT SURVEY INQUIRY
(2) EXIT INTERVIEW SIGN-OFF FORM
The first form, CONFIDENTIAL EXIT SURVEY, is completed by the employee and
returned to the State
Equal Opportunity Office, 1 Capitol Hill, Providence, Rhode Island 02908-5865. This form will not become
part of the employees personnel file. All information obtained from this form will be handled in a confidential
manner and will not be divulged to supervisors, co-workers, or anyone inside or outside the agency.
The second form, EXIT INTERVIEW SIGN-OFF FORM, is to certify that the employee
has received an
Exit Interview Form from the Office of Human Resource Administration. A copy of the Sign-Off form will
be placed in the employee's personnel file. The Confidential Exit Survey will not be place in the employees
Employees who are not able or not available to sign for receipt of the Sign-off
form will have the form signed
by a personnel officer, verifying the employees receipt of the form or that the form was sent to the employee
It is the policy of the University to conduct an interview session between a
representative of the Office
of Human Resource Administration and all NON-CLASSIFIED employees on unlimited appointments who
are scheduled to terminate their employment with the University for any of the following reasons
- LAY-OFF (LACK OF WORK)
- LAY-OFF (LACK OF FUNDS)
- DISCHARGE FOR JUST CAUSE
If for any reason a personal interview is not possible, an EXIT INTERVIEW QUESTIONNAIRE
be sent to the employee.
Terminating/Transferring non-classified employees, upon signing termination or
transfer papers, in the Human
Resource Office, will be given an EXIT INTERVIEW QUESTIONNAIRE.
