It should be noted that this form does not replace the two URI Human Resources incident/injury reporting forms (24 Hour Notification and Incident/Injury Report) that must be submitted if there is a personal injury involved in the incident.  These two forms are located at http://www.uri.edu/hr/forms.html (in the Workers Compensation section).  At GSO, these forms are submitted to Human Resources via the GSO Dean's Office.
