JOINING VARSITY TEAMS AT URI

If you need to contact Coach Westkott about either the Men's or
the Women's Varsity Swimming/Diving Teams you can call him at
his home: (401) 789-6341, at the pool: (401) 874-2005, or write to:

If you contact Coach Westkott and tell him that you are interested in swimming or
diving at URI, he will send you a "Swimming Questionnaire" to fill out and return.
However, you can save a lot of time and hassle by simply clicking on this
SWIMMING QUESTIONNAIRE, and then doing one of these four things:

1. Click open the "Questionnaire."
   • Write down your answers to each question.
   • Enclose your answers in correct order in an E-Mail Letter.
   • Send to ( westkott@uriacc.uri.edu ).
2. Download the "Questionnaire" to your PRINTER
   • Print a copy to send and one to keep for yourself.
   • Fill in the information requested.
   • Send it to Coach Michael Westkott, Intercollegiate Athletics,
     University of Rhode Island, Kingston, RI 02881; OR..
3. Download the "Questionnaire" to your PRINTER
   • Fill in the information requested.
   • FAX it to the URI Athletic Department (401) 874-2458 with a "cover" to Coach Michael Westkott, Swimming and Diving Programs; OR.
4. Download the "Questionnaire" as a file named "swimques.doc"
   • Fill in the information on your computer and save as a ".doc" file.
   • Attach your "swimques.doc" to an E-Mail letter and send
     directly to the Coach at his E-Mail address:
   • westkott@uriacc.uri.edu
