phpESP Survey/Form Tool
phpESP is an online application that lets non-technical users create surveys/forms, administer surveys/forms, gather results, and view statistics.
This page explains the steps you have to take in order to register a phpESP account and to use the phpESP tool to create a survey/form, test it, and incorporate it into a Web page.
Log on - Form/survey designers who already have an account may log on to the administrative interface to start to create forms and/or surveys.
Need an account? - Apply for a form/survey designer account, if you have not already done so.
