phpESP Survey/Form Tutorial

In order to get the most from this package, you should possess some basic knowledge of various form elements. Please visit the form elements tutorial for further explanation of this topic.

This page explains the steps you have to take in order to register a phpESP account and to use the phpESP tool to create a survey/form, test it, and incorporate it into a Web page.

Step 1: Register an account

If you have not already done so, register to create surveys/forms. If numerous people in your department would like to be able to edit or test the survey/form, or view the results, you will be assigned to a group based on your department. You will be notified by email once your account has been created.

Step 2: Create/define a survey/form

After logging in to the phpESP Management Interface, define general information about your surveys/forms.

Step 3: Compose survey/form questions

Using more online forms, add questions to the survey/form.

Step 4: Reorder your questions

Reorder or group similar questions.

Step 5: Deploy survey/form

Link directly to the survey/form or embed the survey code into your HTML page.
